Job summary

Humana Inc. is seeking a Case Manager to serve members in the Wisconsin Family Care program. You will assess health and safety needs, develop a comprehensive Member Care Plan, and coordinate with a Field Care Nurse to deliver services in the member’s home or community.

The role involves frequent travel across Dane/Columbia counties and surrounding areas. Responsibilities include face-to-face assessments, regular visits, care coordination, and thorough documentation.

Qualifications

• Bachelor’s degree in human services or related field; 1 year of relevant experience.
• Or a Bachelor's in another field with at least 3 years of relevant experience.
• Valid state driver’s license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address needs in the least restrictive environment.
• Conduct face-to-face social assessments upon enrollment and at least every six months.
• Conduct quarterly in-person visits and maintain monthly contact with members by phone.
• Arrange non-medical support services (social integration, housing, community resources).
• Ensure cost-effective service delivery and maintain MCP documentation.
