Job summary

Acenda Integrated Health is seeking a dedicated Care Manager to provide integrated case management services, coordinating hospitalization discharge and post-discharge support with community providers. You will link clients to housing, substance use treatment, mental health services, and primary care.

The role requires collaboration with clinicians and community partners, weekly face-to-face client contact, and timely documentation.

Qualifications

• Masters in social work, counseling, psychology or related field.
• BA in behavioral health with 1 year relevant experience or
• Associate's degree in direct care field with 2 years relevant experience or
• High school diploma with lived recovery experience and potential peer certification within 1 year.

Responsibilities

• Provide face-to-face targeted case management, in home or community settings.
• Perform assessment, service planning, linkage, monitoring, and advocacy.
• Maintain documentation within 24 hours of contact; complete treatment plans and assessments.
• Participate in weekly staff meetings and program supervision.
• Coordinate pre-discharge planning and community linkages to support client stability.
