Job summary

Acenda Integrated Health is seeking a Case Manager to collaborate with clients, community services, and clinicians to ensure effective care plans. The role connects clients to housing, substance use services, mental health care, and primary care, with on-call support for crises.

Base pay starts at $20.19/hour plus a $1,000 sign-on bonus. Bilingual Spanish proficiency adds $1.50/hour. On-site work at Gloucester ICMS offices with travel to community sites is required.

Qualifications

• Possess a Master’s Degree in social work, counseling, psychology or related field; or equivalent.
• Bachelor’s degree in a behavioral health field with 1 year of relevant, post-degree experience.
• Associate’s degree in a direct care field with 2 years of relevant post-degree experience.
• High school diploma with lived recovery experience and related credential training within one year.

Responsibilities

• Provide face-to-face targeted case management and community linkages.
• Assist with planning, documentation, and ongoing monitoring of care plans.
• Coordinate hospital discharge planning and post-discharge follow-up.
• Maintain timely documentation and communicate with referral sources and agencies.
