Job summary

Acenda Integrated Health is seeking a Care Manager hourly as part of Integrated Case Management Services - Gloucester. The role collaborates with clients, families and providers to implement effective care plans and coordinate crisis and post-discharge care in the community.

The position emphasizes timely documentation, hospital discharge planning, and strong communication with referral sources, with on-site coverage and flexible scheduling to support client needs.

Qualifications

• Master0? degree in social work, counseling, psychology or related field.
• Bachelor0degree with one year post-degree behavioral health experience.
• Associate0degree with two years direct care experience.
• High school degree with lived recovery and peer certification training within one year.

Responsibilities

• Case manager supports clients with face-to-face targeted case management and community integration.
• Provide assessment, service planning, linkage, ongoing monitoring and advocacy.
• Maintain documentation within state contracts and program manuals for billing.
• Schedule weekly face-to-face contact, coordinate referrals and discharge planning.
• Collaborate with hospitals, community agencies and ROH-level needs.
• Attend program, agency and supervision meetings; complete trainings as required.
• Foster timely communication with referral sources and clients; follow program policies.
• Assist with budget-conscious operations and resource navigation.
