Qualifications

• Bachelor’s degree in a behavioral health, human services, nursing, public health, or related field preferred or equivalent experience.
• Minimum of 2 years of behavioral health care coordination or related service delivery experience.

Responsabilités

• Chart review and documentation in the Care Manager System.
• Coordinate physical, behavioral, and social health services across internal programs and external providers.
• Conduct follow-up calls after hospital discharges within 24 hours and ensure follow-up appointments are scheduled.
