Job summary

Mass General Brigham in Cambridge, MA seeks a full-time case manager to conduct intake interviews, determine medical and psychosocial needs, and coordinate services for patients transitioning from inpatient care. Qualifications

• Bachelor's Degree in a related field required.
• RN, PT, OT, SLP, RT or LCSW/LICSW license required.
• Experience in case management and LTAC setting preferred.

Responsibilities

• Discuss assessment findings with physician/treatment team to maximize outcomes.
• Assist in discharge planning.
• Coordinate discharge and aftercare services.
• Develop post-discharge care plans with the interdisciplinary team.
• Identify resources for post inpatient care.
• Complete intake interviews and determine medical/psychosocial evaluation needs.
• Advocate for entitlements and services.
