Job summary

Mass General Brigham is seeking a full-time case management professional for the Spaulding Rehabilitation Hospital campus in Boston. The role focuses on intake, evaluation coordination, and post-discharge planning within a day-shift schedule; a generous sign-on bonus of up to $10,000 is offered and discussed during interview.

The ideal candidate has 2–3 years of case management experience, an MA license, and a Bachelor’s degree.

Qualifications

• Bachelor’s Degree in a related field is required.
• Experience in case management, utilization review, or discharge planning is preferred.
• Licensure in MA as RN, PT, OT, SLP, RT or LCSW/LICSW is required.

Responsibilities

• Discuss assessment findings with physician/treatment team to maximize outcomes.
• Assist in discharge planning as required.
• Communicate effectively internally and externally.
• Coordinate discharge and aftercare services with guidance from the clinical team.
• Support the team and follow through on requests with timely updates.
• Initiate complete assessments to gather baseline data and identify barriers to post-inpatient care.
