The position requires Massachusetts RN licensure, 5+ years acute care experience and 1 year in inpatient case management, with bilingual English/Spanish preferred.

Qualifications

• Graduate of an approved nursing program with current MA RN registration.
• 5+ years of acute care experience required.
• 1 year of inpatient acute case management experience required.
• Bilingual (English/Spanish) preferred; strong communication skills.
• Excellent organizational skills and ability to manage multiple care plans.

Responsibilities

• Coordinate and insure implementation of the plan of care with an interdisciplinary team.
• Assist with discharge planning and post-discharge follow-up to ensure outcomes.
• Collaborate with departments and payers to manage utilization and ensure appropriate care.
• Identify and address insurance and resource needs affecting planning and throughput.
• Communicate effectively with patients, families, and care teams to optimize care transitions.
