• Educate and advocate for patients and families to coordinate post-discharge care

Responsibilities

• Completes intake interviews and assesses medical and psychosocial needs
• Reviews evaluations and plans with the care team to determine appropriate services
• Coordinates discharge planning and post-inpatient care with the clinical team
• Communicates with physicians, therapists, and payers to ensure timely and appropriate care
• Advocates for resources and entitlements for patients and families
• Ensures compliance with regulatory and accreditation standards
