Job summary

The Center at Centerplace is seeking a dedicated Case Manager to support patient placement referrals, assessments, care plans, and comprehensive documentation. In this in-person role, you will coordinate with hospitals, families, and clinicians to ensure safe and timely transitions and quality care.

Responsibilities include psychosocial assessments, discharge planning, regulatory compliance, and collaboration across departments.

Qualifications

• Minimum 1 year of case management experience.
• Experience with discharge planning and community referrals.
• Knowledge of Medicare/Medicaid basics and patient advocacy.
• Strong documentation and multitasking abilities.

Responsibilities

• Complete psychosocial assessments and maintain resident social histories.
• Coordinate discharge planning with hospitals, families, physicians, and insurers.
• Assist residents with discharge needs and community resources.
• Provide education on Medicare, Medicaid, and available financial assistance.
• Participate in interdisciplinary care conferences and care planning.
• Advocate for resident rights and resolve concerns.
• Maintain accurate documentation in resident records and systems.
