Job summary

Qualifications

• Associate degree preferred in human services or related field.
• Three years of recent experience working with individuals with Intellectual Disabilities.

Responsibilities

• Identify and address individuals' needs as part of a team.
• Assist individuals with activities of independent living: money management, household management, medication management, health appointments and follow‑ups.
• Help individuals access and maintain public benefits including food stamps, Mass Health, Medicare, Social Security, housing, fuel assistance.
• Assist individuals with making community connections to enhance quality of life (Best Buddies, Special Olympics, Senior center, YMCA).
• Supervise and work with direct care staff.
• Collaborate with DDS and other social service agencies such as DCF, MHA, BHN.
