Qualifications

• Three years of human service experience.
• Supervisory experience preferred.
• Moderate physical requirements such as lifting, standing, and stairs.

Responsibilities

• Maintains quality of programming through agency standards and regulations.
• Conducts assessments for treatment planning and ISP development.
• Develops goals in ISP or Treatment Plan and monitors implementation.
• Completes semi-annual progress summaries.
• Reports in HCSIS as required.
• Coordinates services around medical, housing, mental health, and financial needs.
• Provides emotional support and guidance to individuals and families.
• Supervises DSPs: interviews, orientation, training, and evaluations.
• Ensures training needs are met and protocols followed.
• Oversees quality of services and licensing in homes.
• Follows emergency policies and reviews referrals.
• Maintains flexible work schedule.
• Performs other duties as assigned.
