People Incorporated in Taunton is seeking a Sr Case Manager for the Individual Supports IS Program. You will assist the Assistant Division Director, coordinate supports, and provide direct care as needed, including weekend coverage.

The role emphasizes empowerment, advocacy, and dignity for individuals, with responsibilities spanning training, housing, transportation, and collaboration with families and staff.

Qualifications

• Bachelor's degree preferred with three years related experience, one year supervisory experience; or suitable education/experience
• Knowledge of agency policies and procedures
• Counseling duties and ability to manage responsibilities and staff
• Flexible schedule with availability including weekends or holidays
• Valid driver's license and background check required

Responsibilities

• Coordinate individual training programs in self-help skills, social aptitude, behavior management, leisure time use, and advocacy.
• Assist in relocation and housing preferences for individuals and housemates.
• Provide direct care as needed to meet program needs.
• Maintain confidential files and ISP assessments; oversee funds management as needed.
• Communicate with individuals' families, guardians, or other stakeholders professionally.
• Assist in employment-related activities including transportation and reviewing paystubs.
