Qualifications

• Bachelor's degree in social work, psychology, human services, or related field.

Responsibilities

• Coordinates with community resources, refers clients for services, and monitors client involvement in services.
• Performs intake interviews and routine clinical assessments for new referrals.
• Develops, directs and implements client treatment plans and skill training.
• Provides support and education to clients, family members, and other natural supports.
• Distributes medication to clients in the community when necessary.
• Coordinates with crisis and Chapter 51 liaison regarding commitment status and crisis needs.
