Case Manager - Targeted Case Management (TCM) Job Code: 2025-BHS-003 Location: Milwaukee Department: ITCM FT/PT Status: Full Time

Position Summary

This position encompasses the care of people dealing with and recovering from mental illness. Under supervision, the Case Manager will develop a treatment relationship with clients in the program and create and implement individualized treatment plans that include follow‑up, case management, and necessary interventions.

Essential Functions
  • Develop a treatment relationship with consumers and formulate treatment plans for the caseload.
  • Provide psychosocial rehabilitation, symptom monitoring & management, and supportive counseling to consumers on caseload.
  • Monitor client’s psychiatric medication regimen and compliance as directed by the physician.
  • Provide crisis intervention and supportive counseling to consumers who are in crisis.
  • Maintain a treatment‑oriented environment under supervision of the program supervisor and ensure consistency in accomplishing treatment objectives.
  • Coordinate utilization of community resources, day programs, and outside health care providers.
  • Assist clients in financial planning and development of income resources.
  • Coordinate consumer access to services and all entitlement programs.
  • Provide treatment‑related education and liaison with client’s family.
  • Assist client in securing adequate housing and provide prompted training for skills necessary to maintain housing.

Job Qualifications

Minimum Qualifications: Bachelor’s Degree in a behavioral science required, with a preference for social work experience; Master’s Degree encouraged. Must meet all WCS requirements for criminal and caregiver background checks, references, and driving record; valid driver’s license with automobile and sufficient insurance to meet agency requirements.

Knowledge, Skills and Abilities

Good computer skills including Word, Excel and CSP Exchange. Strong organizational skills, ability to multitask, meet deadlines, and some knowledge of mental illness. Ability to work as part of a team and independently.

Working Conditions

Work is performed largely in the community, including apartments, client homes, community locations, and other settings. Hours are during regular business hours but will vary based on consumer and program needs, averaging at least 40 hours per week. The schedule is Monday–Friday, 8:00 a.m.–4:00 p.m.

Physical Demands

Position is mobile with time spent in the community, including transporting clients, home visits, and office work. Requires driving throughout Milwaukee County, walking up and down stairs, and lifting and carrying up to 20 pounds.
