Job summary

North Central Health Care in Wisconsin is hiring a Case Manager for the Lincoln County Youth Team in Merrill. This full‑time, on‑site role supports youth and adults with mental health and substance use needs through assessment, planning, and coordinated care.

The position requires a bachelor’s degree in Human Services or a related field and at least 1 year of case management experience, plus a valid Wisconsin driver’s license.

Qualifications

• Bachelor’s degree in Human Services, Social Work, or a closely related field.
• More than 1 year of experience in case management.
• Valid Wisconsin driver’s license and access to a vehicle.
• Ability to operate vehicle and transport clients as required.
• Preferred Master’s degree and 3+ years of case management experience.

Responsibilities

• Conduct initial and ongoing assessments of clients’ mental health and substance use needs.
• Develop treatment/service plans with identified goals and interventions.
• Coordinate services across psychiatry, primary care, schools, housing, and crisis services.
• Organize multidisciplinary team meetings to ensure cohesive care.
• Advocate for clients to access medical, dental, housing, and community resources.
• Document progress notes, assessments, crisis plans, and referrals.
