Job summary

PAM Health Specialty Hospital of Shreveport is seeking an experienced Director of Case Management (RN) to lead the hospital-wide case management program. The role oversees medical record reviews for proper service utilization, discharge planning, and coordination of care with the interdisciplinary team.

The ideal candidate has 3–5 years in case management, RN licensure in the state, and a BSN/MSN with current BLS certification. IRF/LTACH experience and certification in case management are a plus.

Qualifications

• RN licensure in the state of the hospital.
• BLS certification required; BSN/MSN preferred.
• Three to five years of case management experience; IRF/LTACH preferred.

Responsibilities

• Coordinate hospital-wide case management and review medical records for appropriate care utilization.
• Assist case managers in timely care coordination and discharge planning.
• Oversee admissions screening, daily department operations, and quality of care.
• Conduct fiscal planning and ensure regulatory compliance; drive continuous improvement.
