Job summary

Unity Health Care is seeking a Case Manager to support individuals and families with complex social, medical, and behavioral health needs in Washington, DC. Under the supervision of the Senior Director of Social Services, you will develop individualized case plans, connect clients with community resources, advocate for their needs, and coordinate services with interdisciplinary teams.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related Human Services field required.
• At least three years of experience in community health or social services; internship acceptable.
• Experience with vulnerable populations preferred.
• Outreach experience and knowledge of community resources preferred.

Responsibilities

• Conduct outreach to educate clients about available social services and encourage engagement in medical care and case management.
• Build trusting relationships with clients using person-centered approaches and motivational interviewing.
• Assess housing instability, homelessness, substance use, HIV/AIDS, mental health, domestic violence, pregnancy, family challenges, and other social concerns.
• Develop and maintain individualized case plans addressing identified needs and barriers to care.
• Advise clients and connect them to community resources, housing services, and health insurance enrollment programs.
• Maintain documentation of client encounters and progress notes in electronic health records.
