Job summary

Unity Health Care in Washington, DC is seeking a Clinical Case Manager- Upper Cardozo to provide comprehensive case management for individuals with complex medical, behavioral health, and psychosocial needs. This role supports clients in navigating healthcare and social services within an integrated care setting.

Under the supervision of the Director of Social Services, you will educate clients, develop individualized plans, coordinate care with interdisciplinary teams, and help access benefits

Qualifications

• Master's degree required in Social Work or related field.
• DC licensure as LGSW/LICSW or LGPC/LPC required.
• Minimum 2 years post-master's experience.
• Minimum 4 years in community health, behavioral health, or social services.
• Experience with vulnerable, underserved populations.

Responsibilities

• Conduct outreach to educate clients about social services and increase engagement in medical and behavioral health care.
• Utilize person-centered, strength-based, and motivational interviewing techniques.
• Assess clients' psychosocial, behavioral health, and social service needs.
• Develop and maintain individualized case management plans.
• Assist clients in accessing healthcare, behavioral health treatment, insurance benefits, and community resources.
• Coordinate care with medical providers, behavioral health clinicians, and interdisciplinary teams.
• Maintain accurate documentation in EHR and required systems.
• Participate in case conferences and ongoing training.
