Job summary

Community Connections, Inc. is hiring a PSHP Case Manager in Washington, D.C. This role involves managing a case load of up to 25 residents, providing housing placement and supportive services. The successful candidate will work closely with clients to develop individualized plans and ensure they have access to necessary resources.

Qualifications

• 1 year experience providing case management services required.
• Experience working with adults with serious mental illness and/or adults with substance use.
• Ability to work independently and as part of a team.

Responsibilities

• Manage a case load of up to 25 individual residents.

• Assist clients in acquiring necessary skills for community living.
