Job summary

New Vista of the Bluegrass Inc. in Lexington, KY is seeking a Case Manager to assist clients in gaining access to medical, social, educational, and other services, coordinating care across multiple providers.

The role involves linking clients to resources, developing a Person-Centered Recovery plan, and monitoring progress to ensure services meet needs. The position emphasizes collaboration with treatment teams and community partners, documentation to support care plans, and advocacy for clients

Qualifications

• Bachelor’s degree in psychology, social work or other behavioral science related field.
• 1 year of full-time post-degree experience with adults/children in human services; Master’s may substitute.
• ASL proficiency and SLPIAdvanced rating for Deaf/Hard of Hearing caseload; 1 year of post-degree experience.
• Must complete a state-approved Targeted Case Management certification within six months.

Responsibilities

• Provide direct face-to-face services to link clients to resources and programs.
• Assist clients in developing a Person-Centered Recovery plan of care.
• Provide ongoing monitoring to ensure services are furnished as planned.
• Advocate for clients and their families and coordinate care with multiple providers.
• Collaborate with teams to identify goals and coordinate community resources.
• Document to meet medical necessity and Joint Commission standards.

Education

Bachelor’s degree in psychology, social work or other behavioral science related field

1 year post-degree experience in human services (SMI/SED or Substance Use)

ASL proficiency for Deaf and Hard of Hearing caseload

Targeted Case Management certification within six months
