Responsibilities

• Provide direct services for intake, screening, and referrals to other service lines.
• Participate in ongoing assessments, diagnosis, and collateral treatment.
• Coordinate post-hospitalization follow up care.
• Deliver evidenced-based interventions for mental and behavioral conditions.
• Document all clinical work and maintain the client’s electronic health record.
• Collaborate with care team members and community partners as needed.
• Establish safety and aftercare/transition plans when appropriate.
• Manage client risk and provide crisis services to de-escalate crises.
