Job summary

Qualifications

• At least one year of full-time employment experience post-Bachelor’s working directly with adults or ACT population in a human service setting
• Requires reliable transportation and a valid driver’s license.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Participates in development of client’s service plan, consistent with assessment.
• Arranges for delivery of needed services.
• Monitors client’s progress and performs periodic reassessments.
• Provides psychosocial education to clients to increase understanding.
• Performs crisis assistance, including arrangements for emergency referrals.

Education

Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field approved by Medicaid

Completion of state-required case management/service coordination course within six months of employment
