Job summary

PeaceHealth Springfield Rehabilitation Hospital in Oregon is seeking a dedicated Case Manager to join our brand-new inpatient rehabilitation facility opening in 2026. You will participate in pre-admission screening, perform initial assessments, develop treatment plans, and coordinate care with patients and families to support functional independence.

Required are current RN, LCSW, RT, PT, or SLP licensure in OR, with 2+ years inpatient case management or social work experience; CCM/ARN/ACM/ANCC

Qualifications

• Current RN, LCSW, RT, PT, or SLP licensure in OR.
• Minimum of 2 years social work or case management in inpatient setting.
• Effective oral and written communication in English; additional languages a plus.
• Basic computer skills (Excel, Word, Outlook, PowerPoint).

Responsibilities

• Participate in pre-admission screening as requested.
• Conduct initial assessments and develop individualized treatment plans.
• Provide age-appropriate therapeutic interventions and educate patients.
• Obtain pre-certification and re-certification of insurance coverage as appropriate.
• Document patient care, progress, and continuing needs.
• Maintain professional relationships with patients and families and support client relations.
• Contribute to professional development and adhere to policies.

How you'll contribute

A Case Manager who excels in this role:

• Participate in pre-admission screening as requested
• Conduct initial assessments
• Develop individualized treatment plans
• Provide age-appropriate therapeutic interventions
• Evaluate patient and family response to treatment
• Document the course of patient care including progress made and continuing need for treatment
• Establishes and maintains a therapeutic relationship with patients and families
• Provide patient and family education on tasks and resources that will assist with continued functional independence
• Obtain pre-certification and re-certification of patient's insurance coverage as appropriate according to policy and procedure
• Participate in continuing education/professional development activities
• Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others
• Contribute to client relations and marketing activities
• Maintain competency, licenses/ certifications, and memberships in applicable professional organizations
• Other duties as assigned

What we're looking for

• Current RN, LCSW, RT, PT, SLP licensure in the state of OR.
• Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• Effective oral and written communication skills in English, additional languages preferred.
• Basic computer skills in excel, word, outlook, power point, etc. required.
• Must have good organizational skills, time management skills and analytical ability in order to interpret information and carry out duties independently
• Must be cooperative and have the desire to be a team player.
