Qualifications

Responsibilities

  • Monitor progress and adjust plans to achieve outcomes.
  • Maintain case files and documentation per agency standards.
  • Advocate for clients with providers to access resources.
  • Collaborate with staff and partners to improve service delivery.
  • Provide crisis intervention and client safety support.
  • Prepare regular reports on client progress for leadership.
