• Master’s degree in social work or related field preferred; BA/BS acceptable with experience
• Minimum 3 years in case management or related human services field
• Bilingual in English and Spanish preferred
• Ability to integrate Catholic Social Teaching into service delivery

• Provide strengths-based assessments to identify needs and barriers
• Develop individualized service plans and coordinate housing, employment, childcare, and health supports
• Document thoroughly in case management systems and ensure compliance
• Engage in community outreach and collaboration with faith-based partners
