Job description

Overview

Case Manager - Children's Behavioral Health: JFCS is seeking a compassionate professional to provide case-management services to children receiving behavioral-health services across The Valley. The Child Case Manager also provides guidance and support to the families/caregivers of these youth as needed.

Responsibilities
  1. Provides weekly contact with assigned clients, including home/community visits and attending medication checks.
  2. Assists youth in navigating discharge from treatment settings, finding services as clinically indicated, and working to decrease behaviors to improve stability in the home and community.
  3. Coordinates services and collaborates with involved agencies such as the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  4. Coordinates and facilitates Child & Family Team (CFT) Meetings.
  5. Collaborates with the clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork are completed accurately and promptly.
Qualifications

Meet one of the following education/experience requirements:

  1. High School Diploma/GED with 4 years of Behavioral Health experience.
  2. A.A. Degree with two years of Behavioral Health experience.
  3. Bachelor’s Degree in Social Work, Psychology, or a related field.

Additional qualifications include:

  1. Level 1 Fingerprint Clearance Card without restrictions.
  2. Meet agency requirements for Motor Vehicle Record (MVR) report.
  3. Reliable transportation.
  4. AZ driver’s license.
  5. AZ auto insurance.
