Case Manager - Children's Behavioral Health (Phoenix)

Overview

JFCS is seeking a compassionate professional to work with children receiving behavioral health services, supporting their progress towards self-sufficiency and potential family reunification. The Case Manager provides guidance and ongoing support to the families and caregivers of the youth receiving services.

Responsibilities
  • Provide case management services to children enrolled in the behavioral health system.
  • Maintain weekly contact with assigned clients, including home and community visits, attending medication checks, etc.
  • Assist youth in navigating hospital discharges, finding intensive services as needed, and working to decrease behaviors to improve stability at home and in the community.
  • Coordinate services and collaborate with agencies such as DCS, Probation/Parole, DDD, schools, and other providers.
  • Facilitate Child & Family Team (CFT) Meetings.
  • Complete referrals, prior authorizations, and necessary paperwork for service enrollment.
  • Collaborate with the clinical team to ensure accurate and timely completion of all documentation, including Integrated Service Plans and CFT notes.
Qualifications
  • High School Diploma/GED with 4 years of behavioral health experience, OR
  • Associate’s Degree with 2 years of behavioral health experience, OR
  • Bachelor’s Degree in Social Work, Psychology, or related field.
  • Additional requirements:
  • Level 1 Fingerprint Clearance Card without restrictions
  • Meet agency requirements for MVR report
  • Reliable transportation, AZ driver’s license, and auto insurance
