Job summary

Responsibilities include coordinating services with community agencies, providing transportation, and responding to crises. Evening and weekend hours are required to meet client needs, with a strong emphasis on attendance and teamwork.

Qualifications

• BA/BS degree or four years of equivalent education and/or experience in human services field.
• Reliable transportation and automobile insurance; valid, unrestricted driver's license.
• Prior computer experience and basic computer fluency.
• Demonstrated strong interpersonal skills with diverse publics, consumers, and referral agents.
• Flexibility in assigned working hours.
• Commitment to interdisciplinary and community mental health center approaches.
• Energetic and positive approach to evolving changes and challenges in a complex workplace.

Responsibilities

• Provide case management services, resource acquisition, crisis intervention, and outreach to youth and families.
• Deliver services at client homes, schools, program sites, or other locations as needed.
• Provide transportation to activities or appointments per treatment plan.
• Be available on-call for crises in rotation with staff.
• Maintain regular and predictable attendance.
• Other responsibilities as warranted.
