Qualifications

• BA/BS degree or four years of equivalent education/experience in the human services field (preferred).
• Valid driver's license and proof of current car insurance.
• Working knowledge of computers and basic computer fluency.
• Must achieve competency in the EMR software within 6 months of employment.
• Ability to work independently and with others.
• Flexibility in working hours.
• Commitment to interdisciplinary and community mental health approaches.
• Energetic and positive approach to evolving workplace challenges.

Responsibilities

• Coordinate case management services for adults with recurrent mental health issues.
• Develop individualized treatment goals and aid access to treatment.
• Provide supportive counseling and solutions-focused interventions.
• Offer outreach and crisis intervention for timely access and stabilization.
• Complete strength assessments and identify treatment options.
• Develop and review individualized crisis plans.
• Participate in public information and community liaison activities.
• Assist with housing facilities and clients as needed.
• Maintain regular and predictable attendance.
