Encompass Health invites you to join a Case Manager role focused on coordinating patient care from admission to post-discharge across multiple locations in the U.S. This position emphasizes interdisciplinary collaboration, comprehensive discharge planning, and ongoing documentation to ensure optimal patient outcomes.

The ideal candidate will bring a clinical background (RN/SW/OT/PT/ST), valid licensure, and CCM/ACM certification within two years of hire.

Qualifications

• Must have RN, SW, OT, PT, ST, or Rehabilitation Counseling scope and licensure as applicable.
• CCM® or ACM™ certification required or pursued within two years of entry.
• 2 years of rehabilitation experience preferred.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs.
• Coordinate with team to establish tentative discharge plans and contingencies.
• Participate in planning and executing patient discharge experiences.
• Monitor patient experience including quality, timeliness, service appropriateness, payor relations.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management addendums and required documentation.
