Qualifications

• Requires CCM or ACM certification.
• RN license in state of hospital or compact state.
• Minimum 2+ years in case management, inpatient rehab setting.

Responsibilities

• Performs case management duties at an advanced level.
• Analyzes LOS, costs, outliers, and discharge planning issues.
• Oversees department staff in absence of the DCM.
• Leads interdisciplinary team collaborations with medical staff and leadership.
• Precepts new staff or students in case management.
