Job summary

Bloomington Meadows Hospital seeks a Case Manager to coordinate social services with families, referral sources, physicians and schools. You will ensure post-discharge aftercare planning, arrange family therapy sessions, and provide information and referrals as needed while performing routine wellness checks.

• Two years' experience in a mental health care facility preferred; must have strong communication skills.
