Job summary

CarePlus NJ Inc is seeking a Case Manager to join our Behavioral Health Home in Paramus, NJ. This on-site, part-time role (20 hours) focuses on coordinating care for adults with serious mental illness and chronic medical conditions to improve overall health outcomes.

You will lead psycho-educational groups, provide on-site and outreach services, and collaborate with a multi-disciplinary team to develop and implement comprehensive care plans using evidence-based approaches.

Qualifications

• Bachelor's degree in a related field.
• Two years of experience in health care or behavioral health.
• Valid driver's license and must be insurable by the agency's automobile insurance.

Responsibilities

• Facilitate and coordinate psycho-educational services for consumers and families on wellness topics.
• Provide on-site and outreach case management, including transportation.
• Ensure community supports are in place to assist the consumer.
• Document observations and information in the service record and share with team.
• Assist in intake, ongoing recovery, and discharge assessments and evaluations.
• Collaborate to develop bio-psycho-social assessments and care plans.
• Implement evidence-based interventions (e.g., CBT, MI) to help achieve treatment goals.
• Participate in quality assurance activities and cross-functional teams.

Job Summary:

As a Case Manager in our Behavioral Health Home, you will have the opportunity to work with adults in Bergen County with serious mental illness and chronic medical conditions, with the goal of improving their physical wellbeing through integrated care coordination through the use of various evidence-based modalities (such as CBT, MI, etc.).

What You Will Do:

• Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness
• Offer on-site as well as outreach services (including transport of consumers) and fulfill case management services
• Ensure consumers have community support in place that fulfill a positive corroborative role
• Communicate observations and information about the consumer in the consumer's service record and to team members
• Participates in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
• Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
• Implement evidence-based interventions (such as CBT, MI, etc.) and assessments to help individuals achieve their treatment goals, link to community resources, and provide inter-agency coordination of treatment.
