Job summary

Axiom Care is seeking a full-time, on-site Substance Abuse and Behavioral Health Case Manager in Phoenix, AZ. You will manage a client caseload, develop individualized treatment plans, and document progress in EMR systems while ensuring HIPAA compliance.

Responsibilities include facilitating group sessions, coordinating services, and collaborating with the clinical team to support recovery and community reintegration for diverse populations in our sober living and treatment programs.

Qualifications

• Must be proficient in computer applications and electronic medical record systems.
• Proficient with Microsoft Office Suite or related software.
• One year of case management knowledge with AHCCCS population strongly preferred.
• Active CPR and First Aid certification.
• Able to write professional correspondence and reports with accuracy.

Responsibilities

• Maintain adherence to ethical standards and legal requirements.
• Facilitate and document group sessions focusing on recovery and life skills.
• Carry a caseload and guide clients through program phases.
• Develop client-driven treatment plans tailored to individual needs.
• Secure client information per HIPAA standards.
• Document services within 24 hours in the EMR.
• Identify and report safety and maintenance issues at the facility.
• Transport clients to appointments as needed.
• Use coaching and motivational techniques to help clients achieve goals.
• Perform related tasks as assigned by management.
