Job summary

Sanctuary Recovery Centers is seeking a Case Manager in Phoenix to oversee all aspects of case management for clients with substance use disorders. The role involves interviewing families, assessing needs, determining risk, making referrals, and implementing care plans with ongoing follow-up.

The position requires a bachelor’s degree or relevant experience in social work, psychology, or related fields, plus knowledge of opioid-related health challenges.

Qualifications

• Bachelor's degree or related field in social work, psychology, sociology, counseling, or similar field.
• Experience in substance abuse care and service is preferred.
• Understanding of medical and psychosocial issues related to opioid use and substance abuse disorders.

Responsibilities

• Accept newly-assigned cases after intake and develop an individualized case management plan.
• Develop and maintain relationships with community agencies for wraparound services and referrals.
• Monitor patient progression and help obtain resources to achieve goals.
• Provide regular face-to-face reviews to ensure continuum of care needs are met.
• Maintain up-to-date documentation of service objectives, outcomes, and revisions.
• Prepare case management reports for management and administrators.
• Participate in meetings and trainings to enhance service continuity.
• Ensure patient rights and confidentiality per policies, state laws, and federal regulations.
• Collaborate with staff, community agencies, and family members.
• Uphold organizational values and evidence-based practices in service delivery.
• Attend Clinical Oversight sessions as required.
• Verify files and histories are accurate and organized.
