Job summary

Axiom Care is seeking a licensed behavioral health professional in Phoenix to provide individual, family and group counseling with trauma-informed care.

Responsibilities include coordinating care, developing service plans, maintaining accurate EMR documentation, and participating in interdisciplinary teams while promoting CLAS standards. A master’s degree is required; AZBBHE licensure is preferred, with 2–3 years in behavioral health or substance use.

Qualifications

• Master's degree required in psychology, social work, counseling, or health-related field.
• Licensed with AZBBHE preferred.
• 2–3 years in behavioral health or substance use field preferred.
• Proficient with computers and EMR systems.
• First Aid/CPR certified preferred.

Responsibilities

• Provide individual, family and group counseling with trauma-informed care.
• Coordinate care with stakeholders, funding sources and supports.
• Develop and review clinical documentation in EMR for assigned caseload.
• Oversee accuracy of client records in the EMR.
• Collaborate on individualized service plan goals and team meetings.
• Assess, monitor and refer to appropriate level of care for clients.
• Participate in interdisciplinary treatment team meetings.
• Stay current with updated research and best practices.
• Maintain HIPAA standards for client information.
• Train and supervise interns under clinical leadership.
