Job summary

WestCare Foundation in Caguas, PR is seeking a Full Time Case Manager to coordinate comprehensive behavioral health services. You will support clients with substance use and mental health needs, manage HMIS data, and work within a treatment team to optimize outcomes.

Responsibilities include developing client plans, arranging transportation, coordinating with medical staff, and guiding clients toward entitlements and housing post-discharge.

Qualifications

• Bachelor’s degree in a social service field.
• Understanding of substance abuse and mental health treatment concepts.
• Ability to address needs of clients with co-occurring disorders.
• Valid driver license and basic computer literacy.

Responsibilities

• Maintain understanding of Behavioral Healthcare Home model and treatment principles.
• Participate in treatment team and case management activities; coordinate services.
• Enter HMIS data timely and ensure records are accurate.
• Develop client case management plans with input from clients and teams.
• Provide transportation to appointments and obtain relevant records and prescriptions.
• Monitor medication management and ensure timely renewals; report discrepancies.
• Assist with health insurance enrollment for eligible clients and access entitlements.
• Assist discharge planning and secure post-discharge housing and income.
• Maintain documentation of case management activities and participate in QA activities.
