WestCare seeks a dedicated Behavioral Health Case Manager to ensure clients receive comprehensive medical care and services aiming to optimize substance use, mental health, and overall health outcomes. You will participate in a treatment team, manage case plans, coordinate resources, and support clients through discharge and post-discharge planning, including housing and income stabilization.

Qualifications

• Must understand substance use/mental health treatment concepts.
• Ability to conduct case management assessments and develop plans.
• Strong data entry and documentation skills.

Responsabilités

• Develop and coordinate resources (medical, housing, employment, education, etc.).
• Conduct case management assessments and create client case management plans.
• Provide transportation to appointments and gather required records and prescriptions.
• Monitor and coordinate medical care, including medication management and lab results.
• Assist with discharge planning and securing post-discharge housing and income.
