Job summary

WestCare is seeking a Family Tree Program Case Manager in Caguas, PR to provide comprehensive case management for children, adolescents, and families. You will coordinate behavioral health, medical, educational, and social supports to ensure personalized, culturally responsive care.

In this role you will conduct strengths-based assessments, develop individualized service plans, and collaborate with a multidisciplinary team to promote family resilience and access to community resources.

Qualifications

• Bachelor's degree in social work, psychology, counseling, and/or similar field.
• Six (6) months of professional experience in chemical addiction or mental health counseling preferred.
• Experience with SAMHSA-funded programs preferred.

Responsibilities

• Conduct comprehensive strengths-based assessments of participants and family needs.
• Develop individualized service plans with participants, families, and the team.
• Coordinate behavioral health, substance use, primary care, education, housing, and social supports.
