Peninsula Post Acute is seeking a reliable Case Manager Assistant to provide administrative and care coordination support. You will assist with discharge planning, coordinate services, and maintain resident records while communicating with residents, families, hospitals, and providers.

The ideal candidate will have healthcare exposure, strong organization, and excellent communication skills, with a compassionate approach to assist residents and interdisciplinary teams.

Qualifications

  • Prior healthcare experience in skilled nursing, rehabilitation, social services, or case management preferred.
  • Strong organizational and multitasking abilities.
  • Excellent communication and customer service skills.
  • Dependable with good follow-through and detail orientation.
  • Ability to work with residents, families, and interdisciplinary teams.

Responsibilities

  • Assist Case Managers with daily administrative and coordination duties.
  • Communicate with residents, families, hospitals, and providers.
  • Support discharge planning and post-discharge services coordination.
  • Maintain accurate resident records and documentation.
  • Coordinate referrals, appointments, and transportation arrangements.
  • Ensure timely completion of tasks and confidentiality per HIPAA.

We are looking for someone who takes initiative, follows through on responsibilities, communicates effectively, and can be counted on to support residents, families, and the Case Management team every day.This is an excellent opportunity for an organized professional who enjoys helping others and thrives in a fast-paced healthcare environment.Position OverviewThe Case Manager Assistant provides administrative and care coordination support to the Case Management department, helping ensure residents receive timely services and a smooth transition throughout their stay and discharge process.The ideal candidate is dependable, detail-oriented, professional, and committed to delivering excellent customer service while working collaboratively with residents, families, healthcare providers, and interdisciplinary team members.Key ResponsibilitiesAssist Case Managers with daily administrative and coordination duties.Communicate with residents, families, hospitals, healthcare providers, and community agencies.Support discharge planning and coordination of post-discharge services.Maintain accurate documentation and resident records.Assist with referrals, appointments, transportation arrangements, and follow-up services.Coordinate information between departments to ensure timely completion of tasks.Monitor outstanding items and follow through to completion.Maintain confidentiality and comply with HIPAA and facility policies.Provide professional, compassionate support to residents and their families.Perform additional duties as assigned to support the Case Management department.QualificationsPrevious experience in healthcare, skilled nursing, rehabilitation, social services, case management, or a related field preferred.Strong organizational and multitasking skills.Excellent communication and customer service abilities.Dependable with strong follow-through and attention to detail.Ability to prioritize responsibilities in a fast-paced environment.Proficient with computers and standard office applications.Ability to work effectively with residents, families, and interdisciplinary teams.Positive attitude, strong work ethic, and willingness to assist where needed.Why Join Peninsula Post Acute?At Peninsula Post Acute, we value team members who are compassionate, reliable, and committed to providing exceptional resident care.
