Job summary

NCI Affiliates, Inc. is seeking a dedicated Case Manager for Adults with Disabilities to help individuals achieve vocational and personal goals.

You will supervise Direct Support Professionals, coordinate individualized plans, and promote growth and independence within a collaborative, mission-driven team. Responsibilities include annual reviews, coordinating goals, delivering vocational coaching, assessing needs, and ensuring successful placements.

Qualifications

  • High school diploma or equivalent required.
  • Valid California driver’s license and access to a personal vehicle.
  • Ongoing proof of current state-required auto insurance.
  • Ability to pass fingerprint background checks through the California DOJ and FBI.
  • Strong oral and written communication skills.
  • Ability to work independently with minimal supervision.
  • Effective time management skills.
  • Proficiency with Microsoft Office programs.
  • Comfortable networking and building relationships.
  • Preferred: Previous experience working with individuals with developmental disabilities.
  • Preferred: Ability to motivate and inspire others.
  • Preferred: Flexibility with work schedules and willingness to assist at different sites as needed.
  • Preferred: Supervisory experience.

Responsibilities

  • Maintain a professional and respectful demeanor when interacting with individuals served, community members, and stakeholders.
  • Coordinate and schedule annual reviews with individuals served, identifying both short-term and long-term goals and potential barriers.
  • Provide ongoing instruction and support for vocational growth and training objectives.
  • Oversee individual assessments to determine goals, preferences, and needs, and offer recommendations to ensure these needs are met.
  • Monitor and assess the effectiveness of individual placements to ensure satisfaction and success.
  • Collaborate with management to address worksite challenges and explore new opportunities for individuals served.
  • Encourage and support individuals in making positive behavior changes and achieving personal growth.
  • Respond appropriately to behavior situations in both emergency and non-emergency contexts.
  • Ensure staff receive accurate and timely information regarding individuals served.
  • Train staff on completing data charts and required goal tracking paperwork, and provide quarterly updates.
