Job summary

Qualifications

• Bachelor's degree in Human Services or related field.
• Two years of experience writing and implementing vocational plans in a program setting.
• Experience working with individuals with developmental disabilities.
• CPR and First Aid certification completed within 30 days of hire.
• Clear criminal background check as outlined in DDS licensing requirements (LIC 508).
• Reliable vehicle with CA driver's license and current auto insurance.

Responsibilities

• Review assessment data to develop ISPs.
• Coordinate and attend interdisciplinary team meetings.
• Prepare Semi-Annual Progress Reports and maintain case records.
• Train and support direct care staff on goal implementation and documentation.
• Provide job coaching support and field coverage with clients as needed.
• Ensure confidentiality and regulatory compliance in documentation.
• Monitor client progress and adjust services to evolving needs.
• Serve as liaison between clients, families, and service providers.

About the role

What you'll do

• Coordinate and attend interdisciplinary team meetings
• Prepare Semi-Annual Progress Reports and maintain accurate case records
• Train and support direct care staff on goal implementation and documentation
• Provide job coaching support and field coverage with clients as needed
• Ensure all documentation meets confidentiality guidelines and regulatory standards
• Monitor client progress and adjust services to meet evolving needs
• Serve as a liaison between clients, families, and service providers

Qualifications

• Bachelor's degree required; Human Services field preferred
• Two years of experience writing and implementing vocational plans in a program setting preferred
• Experience working with individuals with developmental disabilities
• CPR and First Aid certification (must be completed within 30 days of hire)
• Clean criminal background check as outlined in DDS licensing requirements (LIC 508)
• Reliable vehicle with valid California driver's license and current auto insurance
• Strong knowledge of behavior modification and human development
• Excellent organizational skills and ability to work independently
• Outstanding written and verbal communication skills in English (Spanish a plus)
• Demonstrated ability to work effectively on an interdisciplinary team
