Job summary
Work Creation Program in Santa Ana, CA, is seeking a Case Manager to support adults with developmental disabilities. You will coordinate services, develop individualized ISPs, and advocate for client independence in a collaborative, field-based setting.
The role requires a Bachelor's in Human Services, 2 years’ experience with vocational plans, and a valid CA driver’s license. Compensation includes full benefits, PTO/holidays, mileage reimbursement, and a 401(k) after 60 days.
Qualifications
- Bachelor's degree required; Human Services field preferred.
- Two years of experience writing and implementing vocational plans in a program setting preferred.
- Experience working with individuals with developmental disabilities.
- CPR and First Aid certification (must be completed within 30 days of hire).
- Clean criminal background check as outlined in DDS licensing requirements (LIC 508).
- Reliable vehicle with valid California driver's license and current auto insurance.
- Strong knowledge of behavior modification and human development.
- Excellent organizational skills and ability to work independently.
- Outstanding written and verbal communication skills in English (Spanish a plus).
- Demonstrated ability to work effectively on an interdisciplinary team.

- Review and analyze assessment data to develop comprehensive ISPs.
- Coordinate and attend interdisciplinary team meetings.
- Prepare Semi-Annual Progress Reports and maintain accurate case records.
- Train and support direct care staff on goal implementation and documentation.
- Provide job coaching support and field coverage with clients as needed.
- Ensure all documentation meets confidentiality guidelines and regulatory standards.
- Monitor client progress and adjust services to meet evolving needs.
- Serve as a liaison between clients, families, and service providers.

About the role
- A full-time professional role supporting adults with developmental disabilities. You'll coordinate services, develop individualized plans, and serve as a key advocate for clients working toward greater independence. Ideal for someone organized, compassionate, and skilled at coordinating services in a collaborative environment. Compensation $25/hr plus $0.70/mile reimbursement; comprehensive benefits (medical, dental, vision), paid time off and holidays, and 401(k) after a 60-day period.
