Developmental Pathways (DP) in Aurora, CO, seeks an Active Case Manager to deliver compassionate, person-centered service and coordinate supports for Medicaid Waiver and State General Fund recipients.
You will conduct in-home assessments, engage with families and providers, and collaborate across the Aurora office and in the field. The role includes remote work, travel within Arapahoe, Douglas and Elbert counties, and a 4-day workweek.
As an Active Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support coordination to individuals receiving Medicaid Waiver and State General Fund services. Your days will be filled with:
Enjoy a 4-day, 40-hour workweek (Monday–Thursday or Tuesday–Friday) with the flexibility to work remotely, in-office, and in the field, to include individual’s homes.
This position requires frequent in-person presence, typically in individual's homes, to support essential responsibilities such as monitorings, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.
