Job summary

Austincc is seeking a Case Manager – Active to manage caseloads under Waiver and State General Fund services in the Case Management Department. The role includes assessments, IDT coordination, and documentation to advocate for service needs.

Position offers a 4-day, 40-hour workweek with remote/hybrid options and in-office collaboration at the Aurora, CO HQ. Starting salary range is hourly, with comprehensive benefits and a PSLF-aligned program.

Qualifications

  • Five years of LTSS experience or equivalent education/experience.
  • Valid Colorado driverfs license and proof of insurance.
  • Meet driving requirements (no suspensions and <=2 tickets in past 3 years).

Responsibilities

  • Schedule, facilitate, document, and complete follow-up for all required meetings.
  • Make collateral contacts with community resources to obtain information or coordinate services.
  • Ensure timely entry and distribution of required documentation to providers, individuals, and families.
  • Provide case management services and support to temporarily assigned individuals; assist with department/team coverage.
  • Coordinate support in emergency and crisis situations.
  • Complete assessments to determine eligibility for services and supports via interviewing, observation, and standardized tools.
  • Maintain knowledge of state/federal regulations regarding LTSS, waivers, and related programs.

Developmental Pathways (DP) has an opportunity for a Case Manager – Active in our Case Management Department. The Case Manager – Active will manage a caseload of individuals receiving Waiver and State General Fund services and maintain documentation needed quarterly and annually. The Case Manager – Active will complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools. The Case Manager – Active will complete the required documentation related to monitoring and monitoring activities to advocate for individual service needs and ensure the effectiveness of services delivered (as authorized by the service plan). The Case Manager - Active will facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs; ensure appropriate interaction and communication; and as appropriate to modify services. Lastly, will provide case management services and support to temporarily assigned individuals; and support with department and team coverage as needed.

This role offers a 4-day, 40-hour workweek, with flexible scheduling options of either Monday-Thursday or Tuesday-Friday. The role includes the flexibility to work remotely, as well as the opportunity to collaborate in-person at our headquarters office in Aurora, CO, and/or at off-site locations.

Five (5) years of experience in the field of Long-Term Services and Supports; or

(LTSS), which includes Developmental Disabilities; or

Some combination of education and relevant experience appropriate to the requirements of the position

Valid Colorado motor vehicle license and provide proof of motor vehicle insurance

Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

One (1) year of experience in developmental disabilities/long-term services and supports

Additional Responsibilities

Ensure the accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to providers, individuals, and family members
