Developmental Pathways (DP) seeks an Active Case Manager to join our Case Management Team in Aurora, CO. You will deliver compassionate, person-centered services to individuals receiving Medicaid Waiver and State General Fund supports, conducting in-home assessments and coordinating service plans.

This hybrid role entails a 4-day, 40-hour workweek with remote work, in-office collaboration, and weekly travel to individuals’ homes within the catchment area.

Qualifications

• Bachelor’s Degree; or LTSS experience
• Valid Colorado driver's license and vehicle insurance
• Meet driving requirements - no suspensions and no more than two tickets in past three years

Responsibilities

• Schedule, facilitate, document, and complete follow-ups for required meetings; disseminate updates to team members
• Ensure accurate, timely entry and distribution of documentation to all necessary staff
• Complete documentation related to monitoring activities; evaluate need and effectiveness of services
• Coordinate support in emergency and crisis situations
• Complete assessments to determine eligibility for services and identify strengths, needs, concerns
• Maintain knowledge of state/federal regulations for LTSS and Medicaid waiver programs
• Make collateral contacts with community resources to secure coordination of services

Developmental Pathways (DP) is seeking Active Case Managers to join our Case Management Team!

As an Active Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support coordination to individuals receiving Medicaid Waiver and State General Fund services. Your days will be filled with:

• Conducting in-home visits to complete assessments, monitor service provision, and facilitate service plan meetings.
• Engaging with families and providers in the community to build trust and foster strong partnerships.
• Collaborating at our Aurora office sharing insights and strategies with your team and other departments to enhance service delivery. Average once per month.
• Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings and collaboration, and more.
• Driving between locations, spending time in the community and at your home or chosen remote workspace.
• This dynamic, hybrid role offers a fast-paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities.

Schedule

Enjoy a 4-day, 40-hour workweek (Monday-Thursday or Tuesday-Friday) with the flexibility to work remote, in-office, and in the field, to include individual's homes.

This position requires frequent in-person presence, typically in individual's homes, to support essential responsibilities such as monitorings, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Requirements

• Bachelor’s Degree; or
• Five (5) years of experience in the field of Long-Term Services and Supports; or
• (LTSS), which includes Developmental Disabilities; or
• Some combination of education and relevant experience appropriate to the requirements of the position
• Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
• Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

• Complete required documentation related to monitoring activities and other targeted case management activities; evaluate the need and effectiveness of services requested and delivered
• Coordinate support in emergency and crisis situations
• Must complete and adhere to mandated reporter training and reporting requirements
• Complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools
• Take ownership of knowledge of state/ federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs and other community resources
• Make collateral contacts with such community resources such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure their assistance
• Responsible for accurately allocating all worked time on a timesheet in accordance with company policies to maintain compliance and data integrity
• Provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed
