Job summary

Athelas Institute, Inc. seeks an Assistant Director of Day Services to oversee case management for individuals in day or employment programs. The role coordinates with community resources and public benefits, ensuring eligibility and updating PCPs with timelines and outcomes.

The position requires onsite presence and includes supporting families, maintaining records, and guiding planning meetings. A bachelor's degree and relevant experience are preferred to advocate for the people we serve.

Qualifications

  • Bachelor's degree or equivalent in related field preferred; 2+ years experience with individuals with disabilities preferred.
  • Strong ability to coordinate with community resources and public benefits.

Responsibilities

  • Provides case management services to individuals receiving day or employment services.
  • Coordinates with community resources, public benefits and natural supports based on assessed needs.
  • Assists individuals in becoming eligible for public benefits and other resources.
  • Maintains files and records for the people we support.
  • Coordinates and facilitates Person Centered Planning meetings with the Coordinators of Community Services.
  • Adheres to timelines for the PCP and coordinates with internal staff.
  • Tracks the PCP through the approval process and communicates status changes to families and staff.
  • Prepares social service summaries and necessary paperwork in conjunction with the PCP and DDA regulations.
  • Advocates for the people we support and safeguards their legal rights.
  • Intervenes in daily crisis situations to ensure appropriate services.
  • Assists with coordination of transportation and notifies caregivers about delays.
  • Occasionally provides transportation to meetings and medical facilities.
  • Attends regular staff and planning meetings.
  • Utilizes agency electronic systems for communication and documentation.
  • Notifies caregivers of incidents per PORII policy, and completes required documentation.
  • Checks emails daily and performs other duties as assigned.
  • Position requires onsite presence; no regular remote work.]

Job description

Department: Assistant Director of Day Services

Essential Job Functions:
  • Provides case management services to individuals receiving day or employment services through the Developmental Disabilities Administration (DDA) and/or Division of Rehabilitation Services (DORS).
  • Coordinates with community resources, public benefits and natural supports based on the assessed needs of the people we support.
  • Assists individuals in becoming eligible for public benefits and other resources.
  • Maintains files and records for the people we support. Request needed records from families, agencies, and other professionals.
  • Coordinates and facilitates Person Centered Planning meetings with the Coordinators of Commtmity Services.
  • Adheres to all required timelines for the Person-Centered Plan (PCP), including coordination with other internal agency staff.
  • Tracks the PCP through the approval process and communicates status changes to families and agency staff.
  • Prepares thorough social service summaries, a review of all needed medical assessments and other necessary paperwork in conjunction with the PCP and in accordance with DDA regulations.
  • Assists families, staff and other team members in understanding the changing needs of the individual. Acts as an advocate for the people we support and safeguards their legal rights.
  • Intervenes in daily crisis situations to assist individual in working through time limited problems, as well as ensuring the most appropriate services are received.
  • Assists with the coordination of transportation for the individual, including completing transmittal forms and notifying caregivers regarding transportation delays when needed.
  • Provides occasional transportation to meetings and medical facilities for individuals we support. Must have reliable transportation.
  • Attends regular meetings, including staff meetings, Person Centered Planning meetings and interdepartmental meetings as required.
  • Staff will be required to utilize agency electronic systems for communication, progress notes, incident report filing, etc.
  • Notifies caregivers of incidents in accordance with the Policy on Reportable Incidents and Investigations (PORII).
  • Completes all State and agency required documentation in accordance applicable policies, regulations and procedures.
  • Check, and respond to emails daily to maintain proper communication
  • Other duties may be assigned by the supervisor of Day services
  • Position requires onsite presence; regular remote work capability is not available.
Education:

Bachelor's degree in psychology, sociology, social work, human services, or other related field and 2+ years experience with individuals with disabilities preferred. Associates Degree will be considered with substantial professional experience.

Training and Experience:

Preferred DOA core competencies, Mandt Systems Training, CPR/ First Aid, MTTP Experience working with people with !DD/DD preferred but not required.

Knowledge of ODA and public benefit systems preferred but not required.

Physical Requirements:

Person must be able to walk, stand, and sit for up to 8 hours. Person must have the ability to lift up to 50 lbs. or more based on assigned supported person (using health and safety lifting/ turning techniques or assistive devices).

Person must be able to successfully demonstrate all Mandt Systems training physical interventions and techniques, to include assisting supported person to standing, providing assistance with ambulation, physical releases, and physical restraints.
