Job summary

Developmental Pathways (DP) in Aurora, CO, seeks an Active Case Manager to deliver compassionate, person-centered service and coordinate supports for Medicaid Waiver and State General Fund recipients.

You will conduct in-home assessments, engage with families and providers, and collaborate across the Aurora office and in the field. The role includes remote work, travel within Arapahoe, Douglas and Elbert counties, and a 4-day workweek.

Qualifications

• Bachelor’s Degree; or five years of experience in LTSS/related field.
• Experience in developmental disabilities or LTSS preferred.
• Valid Colorado driver license and proof of insurance.
• Meet driving requirements with no suspensions and limited tickets in past 3 years.

Responsibilities

• Conduct in-home visits to complete assessments and monitor service provision.
• Engage with families and providers to build partnerships.
• Collaborate at the Aurora office and with other departments.
• Host virtual meetings and coordinate services from home.
• Drive between locations and travel within the catchment area weekly.
