Job summary

Department of Economic Security (DES) in Phoenix invites applications for a Case Manager 2 - SkillBridge Intern. The role supports the Division of Developmental Disabilities, providing services and planning to improve quality of life for Arizonans.

Qualifications

• Knowledge of HIPAA and patient privacy requirements.
• Awareness of medical and behavioral health issues and terminology.
• Knowledge of Person-Centered Service Plan (PCSP) development.
• Familiarity with federal/state statutes and agency policies affecting disability services.
• Understanding principles and practices of case management.

Responsibilities

• Drive a vehicle for on-site member visits and meetings.
• Develop, review, monitor PCSP and document progress.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families.
• Write progress notes and prepare reports and special projects.
